What is an Assessment?

Our goal is to ensure individuals and their loved ones feel informed and supported throughout the admissions process. During the assessment, an Intake Coordinator or Registered Nurse will ask questions about current concerns, including any history of mental health conditions or concerns, as well as medical background. The initial screening helps us determine the most appropriate level of care; however, it is not a full psychiatric evaluation.

Family members or support persons are welcome to be present during the assessment process.

How Do I Schedule an Assessment?

To schedule an assessment, call us at 508-329-6412. One of our team members will ask a few preliminary questions to help determine the next best course of action. Same-day assessments may be available.

You can also request an outpatient assessment using our secure online scheduler.

When Should I Seek an Assessment?

If you or someone you know is experiencing difficulties at home, work, or school due to mental health concerns, such as suicidal thoughts, self-harm, mood instability, hallucinations, anxiety, or any other behavioral health symptoms, please call (508) 329-6412.

How Long Do Assessments Take?

Most assessments take approximately 45 minutes to 1 hour. If inpatient treatment is recommended, two hours or less should be expected for the admission process.
